To file a case to request an issuance of a lost title, you must bring the forms and supporting evidentiary attachments to the St. Joseph County Clerk's Office to file the forms. The Clerk's office will provide you with an official cause number for the Petition and the Order. The filing Fee will be due at that time. Filing Fee is $157.00. Cash, Money Order or Credit Card (a convenience fee will apply when utilizing a credit card).

A hearing will be scheduled. It will be your responsibility to appear at the hearing to have your testimony heard and an Order signed. The Court may require the production of additional information not contemplated by this form, depending on the individual circumstances of each petition.
